These initiatives leverage local resources and social networks to address challenges linked, for instance to socio-economic difficulties and social isolation. However, their implementation can vary significantly within and between countries due to cultural norms, historical contexts, and institutional structures. Understanding these contextual differences is crucial to recognise how communities mobilise their resources to support their residents. This panel discussion will explore programmes from different countries, each highlighting different strategies and follow-up results that address social needs and promote wellbeing.
The Regional Government of Catalonia will present its ‘Neighbourhoods with Future’ project, focusing on innovative methods to address social and urban challenges for neighbourhoods, where there is a high number of residents at risk of social exclusion. The University of West Attica will present one strand of the regional West Attica strategy specially focused on the use of EU funds to improving social services and inclusion of vulnerable populations submitted under Greece’s Recovery and Resilience Plan. Finally, the City of Bruges will introduce delegates to their implementation of the Compassionate Cities initiative, in particular a programme to support relatives in times of grief.

Co-creating family and children’s support requires collaborative efforts among various agencies together with families themselves to address their needs, particularly the needs of families in vulnerable situations. This approach emphasises inclusion, equity, and active participation, aiming to build strong support networks and holistic solutions tailored to the diverse contexts of these families. By fostering partnerships and co-creating initiatives with families themselves, the wellbeing and healthy development of children should be improved. In this discussion we will hear about three innovative programmes which provide evidence on the positive impact of multi-agency family and child support.
The City of Leuven will share an integral long-term and collaborative programme supporting families to access multiple resources to help them be included in their communities. The Faculty of Social Work in Ljubljana will explain the use of an innovative laboratory of co-creation of new programmes and integrated services with families. The Romanian National Authority for the Protection of Children Rights and Adoption (NAPCRA) and UNICEF will discuss the MPS model, showing that the delivery of an integrated package of community-based services for children and their families supported by technology results in reducing poverty, inequality and deprivation.
Fostering a culture of co-creation within family and children’s support services is paramount for effective and sustainable support. By actively involving families in decision-making processes, interventions are better tailored to prioritise the well-being and empowerment of children and their families.

The social services workforce is a fundamental part of social protection systems across the EU and plays a key role in improving people’s lives. The demand for social services, support, and protection is rapidly increasing across Europe and beyond. This increase in demand places significant pressure on public social services and the workforce they employ, whilst also facing budget constraints, recruitment, and retention issues.
Social services professionals should be part of the solution to these challenges by being meaningfully involved in the conversations, plans and programmes put forward to manage the future workforce.
In this session, we will learn about three different approaches to address these social service workforce challenges with a particular focus on how professionals are co-creating these solutions. Northern Ireland’s Social Care Council will present their social care reform, which focuses on co-creating a social care workforce that can deliver for now and into the future. CNFPT, the National Centre for Local Public Administration in France will explain how they are integrating the views of people using social services into practitioners’ training. The National College of Social Workers from Romania (CNASR) will present findings from the first study in Romania on the views of social work practitioners on the profession, and how the results will contribute to improving the voice of professionals to bring about workforce reform.
Building strategies that address workforce challenges is an important way to respond to the social care and support needs of individuals and families in vulnerable situations.

The National Joint Investigative Interview Project was set up to improve the quality of the experience of child victims and witnesses who are required to give evidence, to minimise the risk of re-traumatisation and improve the quality of evidence gathered during forensic interviews of social workers and police officers with children. Through a training programme, interviewers are equipped with enhanced skills and knowledge, particularly on trauma-informed approaches, enabling them to create safe, collaborative environments for children to share their experiences effectively during interviews.

Having realised that families with children with disabilities did not have the right type of support, the Welfare Department of Riga City Council conducted a research study and collaborated with service providers and families to develop and implement a new social programme. TNO, an independent not-for-profit research organisation advising the Dutch Government and private sector on social progress, has undertaken research on the effects of shared decision making on the wellbeing of professionals and people in care. Based on the results, TNO developed the ‘Samen Beslissen’ methodology to be used by municipalities in the implementation of shared decision making for care and support services.
The Consortium of Retirement Homes in Vienna (KWP), Austria, has started pensioners clubs, where they implement a co-production approach in their organisational development and the generation of individual care and support plans.

Research shows that loneliness has a serious impact on older people’s longevity, their physical and mental health and their quality of life. Likewise, the lack of care but also its low quality can further deteriorate older people’s wellbeing and their life quality. The TRAPEZISTAK Programme addresses the vulnerability and risk of social exclusion faced by migrant young people without a family network. It implements a personalised support model through social inclusion pathways and intersectoral cooperation, serving as a ‘one-stop-shop’ to enhance the social inclusion and transition to adulthood of young people, most of them migrants, in the Basque Country. It offers administrative assistance, training for employment and language skills, job placement services, socio-educational programmes, community participation initiatives, and housing support.
The programme has shown positive outcomes, with 606 individuals assisted in the first half of 2023. Successes include high rates of municipal registration and health card acquisition, improved language proficiency, and access to housing. Participants also demonstrate proficient personal skills, healthy lifestyle habits, and strong social and community participation.

Education is a crucial element to build the future of our societies. Investments in this field are essential to allow all children to have access to decent and basic learning conditions. Creating a level-playing field for all pupils is needed to provide strong roots for better development of future generations.
This session will introduce three practices to facilitate a child’s path to education. The first two solutions propose, through the funding of national or regional public authorities, to support families with e-vouchers to access school material or food for children. The third solution targets schools directly through online workshops for school principals to help them access European funds and guidelines to prepare healthy meals in schools. The National Emergency Support Mechanism (NERM), established in 2021 to address the conditions to receive unaccompanied children in Greece, developed an integrated action plan to identify, trace and provide care for unaccompanied children. They also improved coordination among child protection agencies in Greece through common protocols, procedures, and referral pathways to transform the way the needs of unaccompanied children are now addressed.
Since then, NERM has handled 10,210 communications, referred 7,556 unaccompanied children, accommodated 4,281 children, completed 5,222 placements, and traced 67 unaccompanied children living homeless. Qualitatively, NERM’s interventions are recognised as best practice by the EU and international organisations such as the UN, the International Organisation for Migration or the EU Asylum Agency.

This programme targets the long-term unemployed, particularly those who have left education and are not in work or training (NEET). It aims to empower them through awareness raising, collaborative partnerships, individualised support, reduction of dependency, improved access to training and successful reintegration into the labour market.
During their session, Pro Arbeit will begin by highlighting successful best practices for long-term unemployed persons, stressing collaboration with beneficiaries, service providers, and policymakers. In the second part, the presenters will involve the audience in interactive exercises to simulate project implementation across agencies, fostering knowledge sharing, collaboration, and strategy replication. A co-working lab method, specifically designed to collaboratively develop new concepts will be used as the primary learning approach. By sharing successful organisational practices, the presenters aim to inspire other organisations to adopt similar strategies, thereby contributing to positive change within their communities, with a specific focus on enhancing support for long-term unemployed individuals.

The use of lived experience is increasingly recognised as fundamental in social services worldwide, providing valuable cultural insight and knowledge while offering a unique perspective from people using services. In Ireland, especially with Peer Support, lived experience roles are acknowledged as essential for designing and delivering social services. Genio will present the different Peer Support Roles that that are incorporated regionally and nationally in Ireland, including across mental health services, in homelessness services, within the Irish Travelling Community and within the international protection system.
Addressing the needs of these populations requires nuanced approaches, with peer-led initiatives proving crucial. By sharing successful organisational practice, Genio will inspire other organisations to integrate lived experience into their organisational strategies.

The ‘Staircase to Staircase’ programme addresses the multifaceted challenges encountered by vulnerable families in Gellerup, Aarhus. These challenges encompass high unemployment rates, limited education, health issues, and barriers to accessing support services. Through innovative family and employment initiatives, this programme aims to address long-term unemployment and dissatisfaction among Gellerup residents.
Activities include forming an interdisciplinary team to streamline support services, conducting outreach to families in vulnerable situations, and building trust through home visits. Positive outcomes observed include increased parental engagement in education or employment, reduced children school absences, and more adolescents participating in part-time after-school activities. Additionally, parents report better management of their health and well-being as a result of the programme’s interventions.

This programme addresses the social exclusion of people accessing mental health services, often presenting frequent readmissions to hospitals or living in isolation in the community. For instance, it provides early intervention and preventative support to individuals living with mental health conditions in the community while offering discharge support services to reduce repeat hospital admissions. Support Practitioners build relationships and trust with individuals, identifying key barriers and early support needs to ensure their support aligns with their expectations.
A support plan is co-created with the person, and this is flexible, so the person’s support can increase or decrease over time, as appropriate. Carr Gomm’s services have notably facilitated successful hospital discharges, addressed social barriers, and enabled positive transitions to community living. 76% of individuals experiencing successful discharge during the evaluation period received varying levels of support tailored to their individual needs.

Persons with disabilities have the right to live independently and choose where and with whom they want to live. Supporting independent living and inclusion in the community for people with disabilities is one of the key pillars of the EU Strategy for the Rights of Persons with Disabilities 2021-2030 and a cornerstone of the development of community-based care in line with the United Nations Convention on the Rights of People with Disabilities (UN CRPD). Social services across Europe are developing initiatives to ensure independent living for all people with disabilities.
In this session, we will hear about four promising practice examples from Italy, France, Germany and Spain including co-produced personalised care plans, guidance on how professionals can support people’s self-determination, a strategy based on a new role of participation managers to ensuring the accessibility of services, and agencies facilitating access to independent living.

Digital inclusion ensures that individuals and communities, especially those traditionally underserved, have equal opportunities to participate in initiatives powered by technology. Within this context, coproduction emphasises the collaboration of public authorities with service providers companies, research institutes, and the community for instance, in the development of digital solutions. This is an ongoing and dynamic process that requires continuous collaboration, adaptation, and innovation to address the evolving needs of the community and the changing landscape of digital technologies. Collective design, development, and implementation of initiatives to bridge the digital divide ensures that everyone has access to and can benefit from digital technologies.
In this session, delegates will learn about four initiatives promoting digital social inclusion. The University of Hertfordshire (UK) will explore how digital technologies can support social wellbeing by reducing social isolation and increasing links with the community. The city of Ghent (Belgium) will discuss how they promote digital inclusion through digital coaches. The Foundation for Social Welfare Services in Malta will present a national case management system to support professionals in their daily work and have them be more focused on people’s social inclusion. Finally, the Third Sector Platform of Catalonia, Spain, will explain how their newly developed AI radar identifies which automated decision-making mechanisms are currently being implemented by public authorities and which groups are most impacted.

This session will focus on co-creation of effective policy responses to address the multi-faceted challenges faced by the long-term care workforce.
The European Care Strategy calls on Member States to improve working conditions and attract more people – in particular men – to the care sector. Some of the proposals suggested include collective bargaining and social dialogue, ensuring the highest standards of occupational health and safety, designing continuous education and training for care workers, tackling gender stereotypes, and ratifying and implement international conventions on domestic workers.
Participants will learn from proposals put forward by national long-term care strategy coordinators to increase the sector attractiveness, notably through improving working conditions, increasing the professionalisation of care and addressing skills needs.
